How do I complete my medical history form, and why does it matter?

Quick answer: In the app or on the web, under your profile. It is the most important thing you will fill in, because your clinician reads it before treating you.

Your medical history tells the clinician what is safe. Medications, medical conditions and allergies can all change what treatment is appropriate and how it is carried out.

What to include

  • Every medication you take, including over the counter and supplements
  • Medical conditions, including anything being investigated
  • Allergies, particularly to medications and latex
  • Whether you are pregnant or breastfeeding
  • Anything you are anxious about

Blood thinners, pregnancy and ongoing conditions

These do not usually prevent treatment, but the clinician must know in advance so they can plan properly. Never stop or change a medication before dental treatment unless your doctor has specifically told you to.

Keep it current

Update it whenever something changes rather than only before your first visit. It takes a minute and it is the difference between your clinician working with accurate information or out of date information.

Note: You do not need to bring a written list to your appointment. If it is in the form, the clinician has it.
